    Implementation of Diagnostic Criteria for Common Occupational Radiation Diseases in Nuclear Sector and Some Provinces in China

    To evaluate the implementation of common diagnostic criteria for radiation diseases among diagnostic physicians, radiation workers and radiation health supervisors, to understand the scientificity, practicability of these criteria and the problems existing in the implementation process, a questionnaire survey was conducted in nuclear industry and selected provincial regions including Sichuan, Jiangsu, Inner Mongolia, Shandong, Henan, Tianjin, Anhui, Zhejiang. Our questionnaires were divided into volume A, B and C, corresponding to physicians, workers and supervisors respectively. The results showed that:(1) the recovery for A, B and C is 140, 2 815 and 310 respectively;the mastery and awareness rate from high to low are of physicians, supervisors and workers;(2) with the quantitative score of the criterion technical content set to 100, we get 93.1, 92.8, 85.5, 88.7, 87 respectively for the diagnosis of occupational radiation skin diseases, external exposure acute radiation disease,external exposure chronic radiation diseases, radiation tumour and cataract.

    In view of the publicity training, implementation effect and quality of diagnostic criteria for occupational radiation diseases, a questionnaire survey was conducted among physicians, radiation health supervisors and radiation workers in Henan Province, to evaluate their application and make suggestions for their formulation, revision and publicity. A total of 11 physicians,35 supervisors and 595 workers were gotten involved. We find that the training rate of physicians and supervisors are 100%; the mastery rate of physicians is 90.19% to 100% for different criteria. The supervisors' awareness rate of “the Judgement standard of occupational radiogenic neoplasmsis” as low as 54.29%.The correct understanding rate of radiation carcinogens by workers is low, ranging from 58.49% to 64.37%. The feasibility determination and revising suggestions of the criteria by physicians and supervisors are mainly the dose threshold and dose estimation. Workers pay more attention to the diagnosis of occupational radiation diseases, and suggest strengthening training, personal dose monitoring, occupational health examination, and health supervision and law enforcement.

    To evaluate the implementation and application of 5 commonly used diagnostic criteria for radiation diseases in Jiangsu Province, a questionnaire survey was conducted among occupational diagnosis physicians, radiation workers and radiation health supervisors. The results showed that:1) for physicians, the master rate, training rate and adoption rate are 61%-89.6%, 59.7%-92.2%, 62.3%-68.8% respectively. The training rate and adoption rate among them are significantly higher than those among supervisors; 2) there is no significant difference in the awareness among workers and supervisors, except that for “the criterion of occupational radiation skin disease diagnosis”; 3) the training rate of supervisors is 3.2% to 48.0%, which is significantly lower than that of physicians and workers; 4) the Q&A correctness rate of workers on the cognitive status of occupational radiation diseases is 34.7% to 89.7%, and there is no obvious difference between medical and non-medical institutions; the awareness rate of workers on information submitted for determining occupational radiation diseases is 73.6% to 100%; 5) the recognition of some provisions of the criteria by physicians, workers and supervisors is acceptable.

    With questionnaires formulated, we conduct an online survey among occupational disease diagnostic physicians, radiation workers and health supervisors, to track the implementation of criteria for common occupational radiation diseases in Shandong Province. We then discuss the scientificity, rationality and existing problems of these criteria, and provide reference for their revision. This also contributes to the management of radiation health. The results show that the awareness rate is 58.47% to 100%; the feasibility rate of clause judgement is 88.89% to 100%; the recognition rate of compliance with existing regulations is 90%. The judgement accuracy rate and awareness rate of workers on the problems are 74.58%-88.98%, 81.36%-100% respectively.

    In view of the publicity training, implementation effect and quality of 5 diagnostic criteria for common occupational radiation diseases, a questionnaire survey was conducted among radiation workers and radiation health supervisors from China's nuclear industry, to evaluate the publicity, mastery and implementation of them. The results showed that the criterion for “Diagnosis of acute radiation sickness from occupational external exposure” was well awared and trained in both groups. While these criteria's content coordination and feasibility received high marks, the relevant funding matching is average, and the understanding of them still needs to be further improved.

    In the form of questionnaire survey, 6 cases of occupational acute radiation skin injury diagnosed from 2016 to 2019 were collected and statistically analyzed in terms of length of service, type of work, exposure time, exposure dose and clinical manifestations, and the characteristics of occupational exposure, occupational exposure control points and follow-up feedback of applicable diagnostic criteria were analyzed and discussed. The results suggest that industrial radiation and industrial flaw detection are the key jobs leading to occupational acute radiation skin injury. Radiation units should strengthen the training of radiation workers, especially the pre-job training, to improve the awareness of protection. Workers entering radiation workplaces should not only wear conventional dosimeters, but also carry alarm dosimeters. The diagnosis of occupational acute radiation skin injury involves the diagnostic physician's familiarity with the cviteria, but the key to the diagnosis of occupational acute radiation skin injury is to determine the dose and clinical manifestations.

    Through the analysis of 20 cases of occupational radio cataract, we explore its incidence among radiation workers at different periods, different posts, different working conditions and different doses. It's expected to be helpful to promote the evaluation of current diagnostic criteria, as well as the radiation protection and health management of key occupational groups.

    We review the clinical analysis and etiological judgment of 8 cases of lung cancer in uranium mine workers, to further understand the risk of radiation lung cancer among workers exposed to high radon levels, and the scientific nature of probability judgment of its etiology. It is expected to be helpful to strengthen the radiation protection, and analysis and research of related etiological judgment, of underground workers in uranium mines.

    Analyses 8 cases of occupational radiation tumor (leukemia). It shows that all cases were caused by chronic occupational exposure,despite distribution in 6provinces, i.e., Hubei, Heilongjiang, Shandong, Henan, Jiangsu and Anhui. In the end, 3 cases died. Hence, more attention should be paid to personal dose monitoring and health monitoring of radiation workers, protection knowledge education should be strengthened, and a complete database of radiation workers should be established to optimise protection and the accuracy of diagnosis, with a view to discovering the occurrence of occupational radiation tumours.We analyse 8 cases of occupational radiation tumor (leukemia) in China.
